Thank you for your Wyandot Day Camp registration. We look forward to providing a positive and memorable camp experience for your camper. Please read the Day Camp Confirmation below.
Please download, complete and bring the Day Camp Health and Parental Consent forms with you to the first day of camp. Please do not mail these forms to the office.
If you have any questions concerning your child’s participation at Camp Wyandot Day Camp or if you need to have these forms sent to you, please call the Camp Wyandot office at 614-481-8227 or email info@campwyandot.org.
Day Camp Health Form 2020– Read, complete and bring to camp. (A four page form that must be filled out and signed by the parent/guardian. Campers will not be admitted to camp without this completed form.)
Day Camp Parent Consent Form 2020 – Read, complete and bring to camp. (A two page form that gives consent from a parent /guardian for your camper to participate in camp as well as important contact details. This also lists other persons that will have permission to pick up your child at camp. Campers will not be admitted without this completed form.)
